Morgan Stanley initiated coverage on eight biotechnology companies in a recent research report, spanning rare diseases, ophthalmology, pulmonary diseases, and neuropsychiatric disorders. The bank is particularly bullish on PTC Therapeutics' (PTCT.US) growth potential driven by Sephience, and Belite Bio's (BLTE.US) opportunity to become the first approved therapy for Stargardt disease to reach the market. Meanwhile, the bank is watching the market opportunity for Trevi Therapeutics (TRVI.US) in chronic cough, and COMPASS Pathways' (CMPS.US) potential to enter the commercialization stage in 2027. PTC Therapeutics "Overweight" rating, price target $104 Morgan Stanley believes that Sephience, an oral treatment for phenylketonuria (PKU) developed by PTC Therapeutics, has already surpassed $600 million in annualized sales just one year after launch, with peak revenue potentially reaching approximately $1.7 billion. Combined with approximately $2.2 billion in cash and cash equivalents as of June 30, 2026, the stock still has upside even without ascribing any value to the company's pipeline. Currently, all of the approximately 100 initial centers of excellence in the U.S. have begun prescribing Sephience. Morgan Stanley believes that net new patient additions and patient persistence are key points of debate in the market. For the third-quarter results to be announced on October 29, if net new patients are flat versus 403 in the second quarter while the company raises its fiscal 2026 product revenue guidance, the bank would view this as an in-line result. The bank forecasts Sephience fiscal 2026 revenue of $955 million, above the company's current guidance range of $850 million to $950 million. Morgan Stanley ascribes only modest value to the pipeline. Beyond the marketed business, potential catalysts include: communications with the U.S. Food and Drug Administration (FDA) regarding votoplam in the second half of 2026, initiation of the PROVE-FA study, and submission of the ST-920 biologics license application (BLA), expected to be completed in the fourth quarter of 2026. Belite Bio "Overweight" rating, price target $206 Morgan Stanley believes tinlarebant has the potential to become the first approved therapy for Stargardt disease type 1 (STGD1), with a target PDUFA date of February 12, 2027. This is based on the Phase III DRAGON trial showing a 35.7% reduction in lesion growth rate (p=0.0033), with a favorable safety profile consistent with its mechanism of action. Although approval may rest on a single Phase III trial, the bank believes other evidence from the DRAGON trialincluding fellow-eye data, DAF and qAF metricsprovides further support. Additionally, management's characterization of FDA feedback has been relatively positive. The bank does not expect the FDA to require data from the DRAGON II trial. Morgan Stanley sees multiple blockbuster-level market opportunities in STGD1, with risk-adjusted peak revenue of approximately $3 billion. Currently, approximately 20,000 patients in the U.S. have been clinically diagnosed. Final pricing relative to the bank's assumed wholesale acquisition cost (WAC) of $300,000 could also provide additional upside. In the first half of 2027, interim data from the PHOENIX trial in geographic atrophy (GA) will be announced. The bank assigns a 40% probability of success (PoS) to this program, and results could provide further upside beyond the STGD1 business. Trevi Therapeutics "Overweight" rating, price target $23 Morgan Stanley believes that Haduvio's mixed mu-opioid receptor agonist/kappa-opioid receptor antagonist mechanism positions Trevi Therapeutics to become the first provider of an FDA-approved chronic cough therapy. The bank sees a relatively low-risk, blockbuster-level opportunity in idiopathic pulmonary fibrosis (IPF)-related cough that does not depend on the refractory chronic cough (RCC) business. The latter's Phase IIb LAKE trial could further unlock multiple blockbuster-level market opportunities. Morgan Stanley believes that in the Phase III OCEAN-2 trial (data expected in the second half of 2027), a reduction in 24-hour cough frequency of at least approximately 30% versus placebo, supported by patient-reported outcomes (PRO), with discontinuation rates similar to placebo, would constitute an in-line result. This assessment is consistent with the trial's statistical power design assumptions and with the 36.5% reduction observed in the 54 mg twice-daily (BID) arm of the Phase III CORAL trial. Ahead of the data readout, key events to watch include the sample size re-estimation for the RCC indication's LAKE trial (expected in the fourth quarter of 2026) and alignment with the FDA on a non-IPF interstitial lung disease (ILD) indication (expected to advance in the second half of 2026). The OCEAN-1 trial is expected to report data in the first half of 2028, providing an assessment of 24-week durability of efficacy. Compass Pathways "Overweight" rating, price target $18 Morgan Stanley believes COMP360 has the potential to become the first approved classic psychedelic therapy. The company's rolling new drug application (NDA) for treatment-resistant depression (TRD) is expected to be completed in the fourth quarter of 2026, with a National Priority Review Voucher (CNPV) supporting approval in the first half of 2027. The filing is built on two positive Phase III studies: the COMP005 trial showed a 3.6-point greater reduction in Montgomery-sberg Depression Rating Scale (MADRS) scores versus placebo after a single dose; the COMP006 trial showed a 3.8-point greater reduction versus the 1 mg active control after two doses; both with p-values below 0.001. The therapy was well tolerated, with no clinically concerning imbalance in suicidality rates. Given that treatment requires an approximately 6-hour monitored session and that commercialization also requires the U.S. Drug Enforcement Administration (DEA) to reschedule the drug's controlled substance classification, launch execution remains a key point of debate in the market. However, Morgan Stanley believes the existing network of approximately 8,000 Spravato treatment centers, the dedicated CPT code for psychedelic therapy, and management's deliberate strategy of "go slow to go fast" all help support the commercialization process. The bank expects revenue to ramp gradually in the second half of 2027 rather than surge immediately at launch. Morgan Stanley sees a blockbuster-level market opportunity in TRD, with risk-adjusted U.S. peak sales of approximately $1.3 billion. There are approximately 4 million TRD patients in the U.S., while Spravato currently treats only about 100,000. Management expects approximately 2 to 4 dosing sessions per patient per year, above the bank's assumption of approximately 1.75, which could also provide additional upside. The Phase IIb/III COMP202 study in post-traumatic stress disorder (PTSD) is not included in the bank's base case valuation but could provide additional upside beyond the TRD business. 4D Molecular Therapeutics (FDMT.US) "Equal-weight" rating, base case price target range $7 to $16 Morgan Stanley believes that 4D-150, administered as a single intravitreal injection, has the potential to substantially reduce the injection burden of anti-VEGF drugs. Frequent injections are a major factor behind the poor real-world efficacy of these therapies, and 4D-150 may improve patient outcomes without necessarily needing to surpass existing therapies on efficacy. However, the bank believes that durability beyond the first year and the 20-week steroid taper regimen are key factors determining whether the therapy achieves broad adoption. Morgan Stanley believes that in the Phase III 4FRONT-1 trial (data expected in the second quarter of 2027), a result would be in line if 4D-150's best-corrected visual acuity (BCVA) is non-inferior to aflibercept 2 mg dosed every 8 weeks (Q8W), with a margin within the approximately 4-letter non-inferiority boundary; intraocular inflammation (IOI) rates are consistent with the PRISM study at approximately 3%, with mild and transient symptoms; and treatment burden is reduced by more than 70% to 80%. Beyond trial data, Morgan Stanley will also focus on the Phase III trial of suravec, a subretinal injection therapy developed under the AbbVie/Regenxbio collaboration (data expected in the fourth quarter of 2026), and Eli Lilly's Ixo-vec ARTEMIS trial (data expected in the first quarter of 2027). Avalyn Pharma (AVLN.US) "Overweight" rating, price target $57 Morgan Stanley believes inhaled delivery has the potential to address the tolerability issues previously faced by Esbriet and Ofev. For AP01 and AP02, the bank sees an opportunity for both drugs to grow into multi-blockbuster-level products without needing to rely on superior efficacy versus existing therapies. Morgan Stanley believes that in the Phase IIb MIST trial (data expected in the second half of 2027), a result would be in line if there is a significant difference in forced vital capacity (FVC) versus placebo of at least approximately 65 mL, with discontinuation rates similar to placebo. Beyond trial data, key events include Bristol-Myers Squibb Company's ALOFTPPF trial (data expected in early 2027) and the exercise of the PARI option (expected before January 1, 2028). Regenxbio (RGNX.US) "Overweight" rating, price target $18 Morgan Stanley believes Regenxbio has two late-stage, one-time gene therapy value drivers: suravec, developed in collaboration with AbbVie, targeting wet age-related macular degeneration (wet AMD) and diabetic retinopathy (DR); and the wholly owned RGX-202 for Duchenne muscular dystrophy (DMD). The most important near-term inflection point is the readout of pivotal trial data in wet AMD. Meanwhile, with the narrowing of Elevidys' label, the Duchenne muscular dystrophy space leaves room for a second gene therapy. (SRPT is covered by Mike Ulz.) Morgan Stanley believes that if both the ATMOSPHERE and ASCENT trials (data expected in the fourth quarter of 2026) show BCVA meeting non-inferiority criteria, treatment burden reduced by at least 50%, and no drug-related intraocular inflammation (IOI), the results would meet the bar for a positive outcome. The rolling biologics license application (BLA) for RGX-202 is expected to be completed in the first quarter of 2027, and the FDA's review designations for the application are also key events. Both will affect whether the company can obtain accelerated approval in the second half of 2027. Key risks include: procedure-related or inflammation-related safety signals in a Phase III program involving more than 1,200 patients; the FDA requiring randomized controlled trial data for the DMD indication; and financing risk. The company currently expects cash to fund operations into the fourth quarter of 2027 in preparation for a potential launch. Kalaris Therapeutics (KLRS.US) "Overweight" rating, price target $14 Morgan Stanley believes that TH103's heparin-binding design has the potential to provide greater durability than existing anti-VEGF therapies. Given that the current stock price reflects little to no value for TH103, the bank believes the risk-reward is attractive ahead of initial data from the Phase Ib/II MAD study in the first half of 2027. Safety is the most important point of debate currently. Morgan Stanley believes the base case should be: the latest batch of TH103 triggers only transient, mild-to-moderate intraocular inflammation (IOI); visual acuity and ocular anatomy improvements are comparable to approved anti-VEGF therapies; and a substantial proportion of patients achieve a retreatment interval of at least 16 weeks. Beyond trial data, Morgan Stanley will also watch the Phase III trial of suravec, developed under the AbbVie/REGENXBIO collaboration (data expected in the fourth quarter of 2026), and the Phase III 4FRONT-1 trial of 4D-150 (data expected in the second quarter of 2027) as key references for assessing durability of efficacy. Given that the company currently expects cash to fund operations into the fourth quarter of 2027 while the Phase III trial is planned to start by the end of 2027, the company will likely need to raise financing before initiating the trial.
