Investigation report on the Sichuan Huaying giant swing accident released, two individuals transferred to judicial authorities

It was learned from the official website of Huaying City that the accident investigation team of the Huaying Municipal People's Government has released the investigation report on the "53" general fall-from-height accident at the Maliuyan giant swing project in Xikou, Huaying City. The investigation determined that the accident was a general production safety responsibility accident caused by the project operating enterprise's failure to fulfill its primary responsibility for work safety, absent management, failure to effectively identify and control safety risks, and violation of regulations by assigning posts to personnel who had not passed work safety education and training, as well as inadequate safety supervision and loopholes in supervisory inspections by the local government and regulatory authorities. On May 3, 2026, a tourist was injured while experiencing the giant swing project at the Maliuyan Scenic Area in Xikou Town, Huaying City, and died after rescue efforts failed. According to the investigation, the direct cause of the accident was: the operator of the giant swing project failed to strictly implement the operating procedures and failed to correctly operate the release rope, causing the release device of the hook releaser on the sliding load-bearing structure to open prematurely before the sliding load-bearing structure had slid to the normal release position; the daisy chain separated from the hook releaser, and there were no on-site safety precautions against accidental falls, causing the tourist to fall and hit the waterfall rock wall, resulting in the accident. At present, the two persons directly responsible for the accident have been transferred to judicial authorities for handling. Issues concerning the performance of duties by relevant public officials discovered during the accident investigation have been transferred to the disciplinary inspection and supervision authorities for accountability. The enterprise involved and other relevant personnel are to be given administrative penalties by the relevant administrative authorities in accordance with the law.
