Amid blowout earnings and concerns over U.S. procedure volumes, JP Morgan maintains an "Overweight" rating on Intuitive Surgical, Inc. (ISRG.US) and raises its price target to $475.
Intuitive Surgical, Inc. (Intuitive Surgical, ISRG.US), the global leader in surgical Siasun Robot&Automation, released its Q3 2026 report on October 20 U.S. Eastern Time. JP Morgan published a research report the same day, maintaining its "Overweight" rating on the stock and raising its price target to $475. The bank said that within the soft-tissue Siasun Robot&Automation track, which is growing rapidly and still has low penetration, Intuitive Surgical, Inc. is the most favorably positioned company. Data showed that the company's revenue, profit, and gross margin all substantially beat market expectations across the board, but the core metric of U.S. procedure volume only met consensus expectations and failed to reach buy-side institutions' optimistic expectations. The stock came under pressure in after-hours trading following the earnings release.
Earnings comprehensively blew past expectations, hardware installations performed impressively
Earnings data showed that third-quarter total revenue was $3.049 billion. System and equipment revenue was $769 million, up 30% year over year; global da Vinci system installations were 546 units, up 28% year over year, with 315 installations of the new-generation dV5 model, significantly above prior institutional forecasts. Installations in both the U.S. and overseas markets achieved better-than-expected growth, and expansion in the ambulatory surgery center (ASC) channel delivered notable results.
Recurring revenue from consumables and services was also strong. Instrument and accessory (I&A) revenue was $1.8 billion, up 18% year over year; service revenue was $482 million, up 22% year over year. On the profit side, adjusted EPS reached $3, $0.36 above market expectations, an increase of 14%; gross margin rose to 69.9%, and operating margin beat expectations by 230 basis points. The company completed approximately $3 billion of share repurchases in the quarter and raised its full-year gross margin guidance to 68.7%-69.3%. However, GAAP EPS was only $1.49, mainly dragged down by a one-time pre-tax charge of $450 million related to an antitrust settlement.
U.S. procedure volumes fell short of optimistic expectations, overseas markets became the growth engine
Procedure volume is the core KPI determining Intuitive Surgical, Inc.'s valuation. Global da Vinci procedure volume grew 14% year over year, but U.S. procedure growth was only 10%, merely in line with market expectations and 1-2 percentage points below buy-side optimistic expectations, which was also the main reason weighing on the stock. Due to the high base, U.S. system utilization declined sequentially, and the bariatric surgery business saw a low-double-digit decline.
In contrast, overseas markets performed strongly, with da Vinci overseas procedure volume up 20% year over year, with broad-based growth across Europe and Asia. India grew as much as 42%, and Japan accelerated for two consecutive quarters benefiting from improved reimbursement policies. China business growth was in line with the company's overall average, but was constrained by a weak capital installation environment. Management narrowed its full-year da Vinci procedure growth guidance to 14%-15%.
Pricing adjustment impact manageable, institutions offer buy-on-pullback strategy
The company introduced a new pricing scheme to extend instrument usage, combined with price cuts in the Chinese market. After full implementation of the policies, revenue per procedure from consumables is expected to face 3-5% pressure, a smaller impact than the 7% level in 2020. The pricing adjustment will be implemented in phases, with the full impact gradually emerging only in 2027. Management, referring to historical experience, believes that price reductions are expected to unlock demand elasticity, and cost cuts can offset gross margin pressure.
JP Morgan noted that 74% of the stock's volatility comes from the company's own fundamentals. Although the overall quality of the earnings report was high, the stock price had already been pushed up by optimistic third-party data, and a reasonable pullback of 3-5% is expected; if the stock falls further by about 7% at the open, that would be a good buying window. In the long term, penetration in the soft-tissue surgery Siasun Robot&Automation track remains low, and combined with new indications and overseas market expansion, the company's growth space remains broad, with risks concentrated in new product R&D, competition from rival products, and product safety incidents.
As of the close on October 20, Intuitive Surgical, Inc. was at $438, with a market capitalization of approximately $158.8 billion and a 52-week price range of $328.57-$603.88.
