Alkermes Public Limited Company (ALKS.US) announced at the Sleep Medicine Annual Meeting in Baltimore on Wednesday that its experimental drug alixorexton significantly improved wakefulness maintenance and reduced daytime excessive sleepiness in patients with narcolepsy type 2. This finding suggests that the highly anticipated orexin receptor agonist class of new drugs may benefit a wider range of patients than originally expected. This study is one of the first large clinical trials to confirm the effectiveness of such drugs for narcolepsy type 2. Current treatment options mainly rely on stimulants or sleep-promoting drugs, which not only have limited efficacy but also often fail to fully relieve all symptoms. This has prompted major pharmaceutical companies to compete in developing more promising treatment options to cover other sleep-wake disorders. Alkermes Public Limited Company CEO Richard Pops said in an interview, "This is the first time we have a tool that is not a 'blunt instrument' to treat narcolepsy. We are directly targeting the neural circuits in the brain that drive the awake state." The orexin system was discovered in the late 1990s and is one of the core mechanisms in the brain that regulates wakefulness. Classic narcolepsy type 1 patients lose neurons that produce orexin. However, developing drugs that can enter the brain and safely mimic the action of this neuropeptide has been a technical challenge for many years. Now, the huge biological potential of the orexin system is fueling a high-risk pharmaceutical competition, with participants including Eli Lilly and Company (LLY.US) and Takeda Pharmaceutical Co. Ltd. Earlier this year, Eli Lilly invested up to $7.8 billion to acquire Centessa Pharmaceuticals, betting that this scientific mechanism could eventually be expanded to a range of neurological disorders. Takeda currently leads in this field, with its leading drug oveporexton for another type of narcolepsy showing impressive results in late-stage clinical trials. New data released at the conference this week showed that the benefits of this drug have extended beyond wakefulness maintenance itself, improving cognitive function, daily functioning, and nighttime sleep quality. In February of this year, oveporexton was granted priority review by the U.S. Food and Drug Administration (FDA), and a decision on approval is expected in the second half of this year. Alkermes Public Limited Company's mid-stage study included 93 patients with narcolepsy type 2 - a subtype that does not have the typical severe orexin deficiency seen in narcolepsy type 1. The company stated that after eight weeks of treatment, patients receiving alixorexton had significantly longer wakefulness on standardized wakefulness maintenance tests compared to the placebo group, and self-reported daytime sleepiness was significantly reduced. Insomnia and urinary frequency were the most common side effects. Alkermes Public Limited Company is betting that orexin mimicking drugs can be used for a wider range of indications. The company is also conducting clinical trials of alixorexton for the treatment of idiopathic hypersomnia (another disease characterized by excessive sleepiness). In addition, the company is researching other candidate drugs based on orexin mechanisms for attention deficit hyperactivity disorder (ADHD) and fatigue symptoms associated with multiple sclerosis and Parkinson's disease. Richard Pops, 64, has been CEO of Alkermes Public Limited Company since the early 1990s and will retire at the end of July this year, continuing to serve as chairman of the board. Pops had previously been considered a potential candidate for FDA commissioner - a position that has been vacant since former commissioner Scott Gottlieb left in May. When asked if he would consider a government position, Pops said he "always leans towards public service" and noted that he has been deeply involved in FDA-related matters for 25 years, but his current focus remains on Alkermes Public Limited Company.
